Support Coordinator
AdaptAbility Support Coordination will work alongside you to help you understand your Plan, and ensure that you exercise and maintain Choice and Control to curate a person-centred network of support.
The role of a Support Coordinator is essentially to ensure that you can achieve your best life, working collaboratively with you to find the best solutions to assist you to meet your goals. A Support Coordinator helps you to understand and to navigate your NDIS Plan, to access and to implement the required services and to connect you to your community. Importantly, a Support Coordinator will help you build capacity so that you can in time, manage and direct your own supports, working with you to achieve your goals and your independence.

My undertaking to you is to improve your long term outcomes and to work together with you to identify and access appropriate options for funded, mainstream and informal supports by creating a cohesive network of providers.
I place an enormous value on individual Participant engagement. My role is to ensure that you obtain the best service available through regular contact with you and proactive engagement with your service providers.
Throughout each step of the NDIS process, I will ensure that you are supported and empowered, that your goals are achievable and fit for purpose and that you can access the best available supports to work towards and to achieve your goals.
